What to Expect on the Day of LASIK Surgery: Hour-by-Hour Guide

    Jan 17, 20265 min read

The biggest source of LASIK anxiety is fear of the unknown. 'Will it hurt?' 'Will I see the laser?' 'How long will I be there?' This hour-by-hour guide demystifies LASIK day so you arrive prepared and leave with the best possible result.

Arrival and Final Pre-Op (45-60 minutes before surgery)

Check-in: Standard registration, ID verification, payment confirmation if not pre-paid.

Final consent: You'll review and sign surgical consent forms after a final discussion with the surgeon.

Pre-op tests: Quick re-verification of measurements, eye pressure, and final corneal scans.

Mild relaxant (optional): Some centres offer a mild oral sedative (like alprazolam 0.25 mg) for anxious patients. This is not anaesthesia — you stay awake and alert.

Numbing drops: Anaesthetic eye drops applied to both eyes 5-10 minutes before surgery. You'll feel a brief sting, then numbness.

In the Operating Room (15-20 minutes total)

Lying down: You'll lie on your back on a comfortable bed under the laser. The bed slides smoothly into position.

Eye preparation: A small device gently holds your eyelids open (like a tiny speculum). You can't blink, which prevents accidents. Some patients find this slightly uncomfortable for 30-60 seconds.

Step 1 — Femtosecond laser (creates the corneal flap): A suction ring is placed on your eye for 20-30 seconds. You'll feel mild pressure and your vision dims briefly. The laser fires for under 30 seconds.

Step 2 — Excimer laser (reshapes the cornea): The flap is gently lifted (you'll see colours and lights — totally normal). The excimer laser fires for 10-50 seconds depending on prescription. You'll hear a clicking sound and may smell a faint burning odour (vaporised tissue — harmless). The flap is repositioned, smoothed, and the surgery is done.

Total time per eye: 5-10 minutes. Both eyes: 10-20 minutes total.

    Immediately After Surgery (15-30 minutes)

    • Vision is blurry, like looking through misty glass
    • Mild irritation, watering, light sensitivity
    • Surgeon does a quick post-op check (slit lamp examination)
    • You receive a clear protective shield (or tape) to wear over your eyes
    • Lubricating drops are administered
    • Brief post-op instructions are given

Most patients are ready to leave within 30-45 minutes after surgery.

    First 4-6 Hours After LASIK

    • Best activity: rest at home with eyes closed
    • Take a 2-3 hour nap if possible
    • Light sensitivity: wear sunglasses if going outside or in bright rooms
    • Mild burning, gritty sensation: normal, lasts 4-6 hours
    • Vision starts clearing within 4-6 hours — most patients see noticeably better than they did with glasses by evening
    • Do: Rest, use lubricating drops every 30 mins
    • Don't: Touch or rub eyes, watch TV/screens for first 2-3 hours, get water in your eyes

    First Night and Next Morning

Sleep: With the protective shield on. Sleep on your back or unoperated side.

Drops: Begin antibiotic and anti-inflammatory drops as prescribed.

Morning: Most patients wake up with clear vision — better than glasses for many. Some mild blurriness from corneal swelling is normal and clears within 1-3 days.

Day-1 follow-up: Mandatory visit for the surgeon to check the flap, measure vision, and start post-op drop schedule.

    Expert Insight

After 20+ years of LASIK, the universal patient feedback is the same: 'That was way easier than I expected.' The actual surgery time is 5-10 minutes per eye. The discomfort is mild. There's no pain — only mild pressure and a few seconds of dimmed vision. The hardest part for most patients is mental anticipation, not the procedure itself. Take a deep breath, follow instructions, and you'll be celebrating your new vision by tomorrow morning.
