Astigmatism: Causes, Symptoms & All Treatment Options Explained

    Feb 22, 20266 min read

If your glasses prescription has a 'cyl' value, you have astigmatism. Roughly 40% of Indian adults do — though most don't know what it actually means. Astigmatism causes blurred vision at all distances, eye strain, and headaches. The good news: it's highly treatable, including a permanent cure for most patients. Here's everything you need to understand.

    What Astigmatism Actually Is

Astigmatism happens when the cornea (or sometimes the natural lens) is shaped more like a rugby ball than a perfect basketball. Light rays don't focus to a single point on the retina — they scatter into multiple focal points, causing blurred or distorted vision.

Unlike myopia (only distance is blurred) or hyperopia (only near is blurred), astigmatism affects all distances.

    Symptoms of Astigmatism

    Common signs include:

    • Blurred or distorted vision at all distances
    • Headaches, especially after reading or screen work
    • Eye strain and fatigue
    • Squinting to see clearly
    • Double vision in one eye
    • Difficulty seeing at night, especially driving
    • Halos and starbursts around lights

Mild astigmatism (under -0.75 cyl) often goes unnoticed but causes subtle eye strain. Higher amounts (-1.5 cyl and above) significantly affect daily life.

    What Causes Astigmatism?

Most astigmatism is inherited — you're born with it or develop it during early growth. Other causes:

    • Genetic predisposition — runs in families
    • Eye injuries or surgeries
    • Keratoconus — progressive corneal thinning that causes irregular astigmatism
    • Constant eye rubbing (especially in children with allergies)
    • Eyelid pressure — chalazion, ptosis can distort the cornea

Astigmatism is NOT caused by reading in dim light, watching TV close, or wearing wrong glasses (common myths).

    Treatment Options

Glasses with cylindrical power: The simplest correction. Modern spec lenses correct any amount of astigmatism.

Toric contact lenses: Special soft contacts designed for astigmatism. More expensive than regular contacts but provide good correction. Need to be properly fitted.

LASIK: Corrects astigmatism up to -5.0 D safely and permanently. Femto LASIK and Contoura Vision both work excellently. Recovery in 24-48 hours.

ReLEx SMILE: Corrects astigmatism up to -5.0 D. Good option if you also want to avoid flap-related restrictions.

Toric ICL: For high astigmatism beyond LASIK limits, or thin corneas. Implantable lens with built-in astigmatism correction.

Toric IOL (during cataract surgery): Permanently corrects astigmatism while removing cataract. Highly recommended if you have both.

When Astigmatism Is a Warning Sign

Most astigmatism is harmless, but rapid changes or unusual patterns can signal:

    • Keratoconus: Progressive corneal thinning. If your astigmatism prescription is increasing every 6-12 months, especially in your teens or 20s, get a corneal topography scan. Early keratoconus can be halted with corneal cross-linking (CXL).
    • Eye injury: Sudden astigmatism after trauma needs evaluation.
    • Pterygium: A growth on the eye surface can distort the cornea — surgical removal corrects it.

Annual eye exams catch these issues early, when they're most treatable.

    Expert Insight

We routinely correct astigmatism up to -5.0 D with LASIK, with results indistinguishable from low-prescription patients. The biggest mistake patients make is assuming astigmatism is 'untreatable' or 'too risky for LASIK' — outdated advice from 15-20 years ago. Modern wavefront-guided and Contoura LASIK platforms handle astigmatism beautifully. If you have stable cylindrical power and healthy corneas, you're a candidate.
