ReLEx SMILE vs LASIK: Should You Choose the Newer Technology?
ReLEx SMILE (Small Incision Lenticule Extraction) is the newest mainstream refractive surgery technology, often marketed as 'next-generation LASIK.' Performed using the Zeiss VisuMax femtosecond laser, it's flapless, minimally invasive, and ideal for active lifestyles. But is it actually better than modern LASIK? Here's an honest, detailed comparison.
How Each Procedure Works
LASIK (Femto/Contoura): A femtosecond laser creates a thin corneal flap. The flap is lifted, an excimer laser reshapes the underlying cornea, then the flap is repositioned. Healing is fast.
ReLEx SMILE: A femtosecond laser creates a small lens-shaped piece (lenticule) inside the cornea, then a 2-4 mm 'keyhole' incision through which the lenticule is removed. No flap is created. The cornea remains structurally stronger.
SMILE uses ONE laser; LASIK uses two (femtosecond + excimer).
Side-by-Side Comparison
Procedure time: - LASIK: 10-12 minutes both eyes - SMILE: 10-15 minutes both eyes
Visual recovery: - LASIK: Sharp vision in 4-6 hours - SMILE: Sharp vision in 24-48 hours (slightly slower initially)
Return to work: - LASIK: 24-48 hours - SMILE: 24-48 hours
Return to contact sports: - LASIK: 4-6 weeks - SMILE: 1-2 weeks (no flap to displace)
Dry eye risk: - LASIK: 5-8% chronic at 1 year - SMILE: 2-4% chronic at 1 year
Cost (both eyes, India 2026): - Femto LASIK: ₹50,000-₹65,000 - Contoura LASIK: ₹1,40,000-₹1,90,000 - ReLEx SMILE: ₹1,70,000-₹2,20,000
Where SMILE Wins
Better for active lifestyles: No flap means no risk of flap displacement from trauma, contact sports, or eye rubbing. Ideal for boxing, martial arts, military, defence, athletics.
Lower dry eye rates: SMILE preserves more corneal nerves than LASIK, leading to less long-term dry eye.
Stronger cornea biomechanically: Less corneal weakening since no flap is created. Important for borderline-thin corneas.
Faster return to high-impact activities: 1-2 weeks vs 4-6 weeks for LASIK.
Smaller incision: 2-4 mm vs 18-22 mm for LASIK flap.
Where LASIK Wins
Faster initial visual recovery: Sharp vision in 4-6 hours vs 24-48 hours.
Treats wider range of prescriptions: LASIK can treat hyperopia (+5.0 D) and very high astigmatism (-5.0 D); SMILE primarily treats myopia (-1.0 to -10.0 D) and astigmatism up to -5.0 D. Newer SMILE platforms (VISUMAX 800) treat hyperopia in some markets but not yet widely in India.
Cost: Standard Femto LASIK is ₹1+ lakh cheaper than SMILE.
Topography-guided customisation (Contoura): Currently more advanced than SMILE customisation options.
Easier enhancement procedures: If a touch-up is needed later, lifting the LASIK flap is simpler than re-treating after SMILE.
Longer track record: 25+ years of LASIK data vs 12 years of SMILE.
Who Should Choose Which
Choose ReLEx SMILE if: - You play contact sports (boxing, MMA, wrestling, rugby) - You're in defence forces, police, or active military - You have moderate dry eyes pre-surgery - You have borderline corneal thickness - Your lifestyle involves a lot of eye rubbing or trauma risk
Choose Contoura/Femto LASIK if: - You want the fastest possible visual recovery - You have hyperopia or very high astigmatism - Cost is a meaningful factor - You want maximum customisation (Contoura's 22,000-point mapping) - You're a busy professional who needs to return to work fast
A proper consultation determines what's medically right for your eyes.
Expert Insight
Both procedures deliver excellent visual outcomes. The 'SMILE is newer, so it must be better' narrative oversimplifies. SMILE has clear advantages for specific use cases — active lifestyles, contact sports, dry-eye-prone patients. But for the average urban professional with healthy eyes and moderate prescription, Contoura LASIK delivers equivalent or sharper vision at lower cost with faster recovery. We recommend both at our centres, purely based on what each patient's eyes and lifestyle need.
